Introduction
D-Ribose is the pentose sugar component of ATP, ADP, AMP, RNA, NAD, and FAD. It is synthesized in the body from glucose via the pentose phosphate pathway, but the conversion is slow relative to demand during repeated high-intensity exercise or ischemic recovery.
Clinical use in cardiology emerged in the 1990s for congestive heart failure and post-ischemic myocardial recovery, where oral D-ribose was shown to accelerate restoration of intracellular adenine nucleotide pools. Sports-nutrition applications followed in the 2000s.
Commercial production is by fermentation of glucose using engineered Bacillus subtilis strains, yielding food-grade and pharmaceutical-grade material with consistent optical purity and low impurity profile.
Typical adult dosing is 3 grams to 5 grams per dose, taken one to three times daily depending on application. The compound is well tolerated, with mild gastrointestinal effects reported only at single doses above 10 grams.
