01 — Overview

Description

A six-carbon aldohexose monosaccharide and epimer of glucose. Approximately 60 percent of the sweetness of sucrose with a clean, slightly bitter undertone. Present naturally in cranberries, apples, peaches, oranges, blueberries, and many plant glycoproteins.

White crystalline powder. Highly water-soluble. Most commonly purchased today not as a sweetener but as a nutraceutical for urinary tract health, where the molecule binds fimbrial adhesins on uropathogenic E. coli and assists clearance.

02 — Background

Introduction

D-Mannose was characterized as a distinct sugar in the late 19th century and named after the manna ash tree exudate from which it was originally isolated. It is structurally closely related to glucose, differing only in the configuration at the second carbon.

Industrial production today is predominantly from D-fructose or from hydrolysis of plant gums and hemicelluloses such as ivory nut, coffee bean, and konjac. The most common commercial route is enzymatic or chemical isomerization of D-fructose, followed by chromatographic separation and crystallization.

Regulated as a food ingredient and dietary supplement in the United States under DSHEA, recognized as a Novel Food where applicable in the EU, and not assigned a separate E-number. JECFA has not set a numerical Acceptable Daily Intake limit.

The unique commercial position of D-Mannose rests on its mechanism of action in the urinary tract. Approximately 90 percent of urinary tract infections are caused by uropathogenic Escherichia coli, which adhere to the bladder epithelium via type 1 fimbriae bearing FimH lectin. D-Mannose is the natural ligand of FimH. Oral dosing at 1 to 3 grams loads the urine with free mannose, which competitively binds bacterial fimbriae and assists their clearance during urination.

Clinical evidence supports use of D-Mannose for prevention of recurrent UTIs in adult women, and the nutraceutical category has grown rapidly since 2015. The molecule is also a critical substrate in congenital disorders of glycosylation type 1b (CDG-1b), where oral D-Mannose is a corrective therapy.
