Introduction
Shiitake has been used as both food and medicine in East Asian cultures for over 1,000 years. The mushroom is the second-most-cultivated edible mushroom worldwide and the beta-glucan lentinan was isolated in 1969 in Japan.
Industrial production proceeds by hot-water extraction of dried Shiitake fruiting bodies or mycelium, followed by ethanol precipitation of polysaccharides, chromatographic purification, and spray-drying. Pharmaceutical-grade lentinan undergoes additional purification for parenteral applications.
Recognized as a permitted food ingredient by the U.S. FDA (GRAS), the European Food Safety Authority, and equivalent regulators worldwide. Lentinan is approved as an injectable adjunct cancer therapy in Japan since 1985.
The beta-glucan polysaccharides activate innate immune system pattern-recognition receptors including Dectin-1 and Toll-like receptor 4, supporting the immune-modulation positioning. Clinical evidence for oral supplementation is moderate; the injectable pharmaceutical application has stronger clinical evidence for cancer adjunct therapy.
